Detection

Risk

Weight accumulates across a session into a band: low, medium, high, critical. Risk is arithmetic you can inspect, not a judgement.

Decision

A person accepts or dismisses each violation, then records one decision with a reason: approve, warning, manual review, invalidate or disqualify. It is attributable, reversible and kept in full history.
